  • You should wear gym attire or loose fit clothing to allow for proper movement.

  • Perspective Physical Therapy and Performance is not a participating provider with any insurance companies. Why is that? The answer is quite simple - we would not be able to provide the highly individualized, one-on-one care that we do at the rates insurance companies currently reimburse for physical therapy services. We see this as a great opportunity for you which we explain below!

  • While cash is certainly an acceptable form of payment, patients are also able to pay with major credit/debit cards, check as well as FSA and HSA accounts.

  • $199 per 1-hour session

    $180 per session, 10-visit package ($1,800)

    $160 per session, 20-visit package ($3,200)

  • This depends on the insurance you have but the answer is yes. Clients seen for physical therapy can send in self-claims to their insurance. You should be able to print claim forms off your insurance company’s website, and send it in with the needed receipts and treatment codes that will be provided upon request at the clinic. The amount of reimbursement or payment toward your deductible is completely dependent on your insurance plan. If you are planning to submit for reimbursement you should call your insurance company prior to visiting Perspective Physical Therapy and Performance. Ask about reimbursement for out-of-network physical therapy expenses, and make sure there are no other requirements like a pre-authorization before you get started.

  • You do NOT need a prescription or referral from a doctor to come to physical therapy. Under New York State law, a script is required after 10 visits or 30 days, whichever comes first. Because of our model, we will typically hit 30 days before 10 visits. Your therapist will discuss this with you and make sure you have a plan to continue with physical therapy.

  • The No Surprises Act is a recently passed law that entitles self-pay patients (not submitting to insurance) to a good faith estimate of their costs , upon request, prior to treatment. While it is difficult to estimate your total costs prior to seeing you, if you request an estimate prior to your initial evaluation, we will provide one to you.

out-of-Pocket vs Insurance Based Physical Therapy

Out Of Pocket Physical Therapy

At Perspective Physical Therapy and Performance, you will spend the entire hour with a Doctor of Physical Therapy. No aides, no techs, no jumping between patients while you wait 5-10 minutes for your physical therapist to return to tell you the next exercise. You will typically be seen one visit per week (maybe two depending on what we both decide is best for you) over a 1-3 month time frame on average.

We believe this is a better way to deliver the best possible care, as it will save you both time and money.

Insurance Based Physical Therapy

An average patient is asked to see a PT 2-3x per week for 6 weeks. At each visit you will most likely have a co-pay ranging from $10-$60. You may have to pay 100% of the costs until your meet your deductible and pay a portion of the cost (co-insurance) until you reach your out of pocket maximum (on average $8,000). You may be assisted by aides or techs and your physical therapist will likely be working with 1-2 other patients at the same time.

This requires more time for you to schedule appointments within your busy schedule and potentially spend more money throughout your plan of care.
